Dear Mr Panigrahi


Further to my e-mail of 23 August, I wanted to apologize to you as I gave you some incorrect information in that e-mail. Your e-mail asked for information about the Active Review process and my e-mail referred to initial interventions, I am sorry for this mistake and I can confirm you are not awaiting initial interventions.


I have liaised with one of the managers at Medway Community Mental Health Team and we appreciate that you were asking for information about the Active Review process ahead of your appointment with Dr Odesanya, but I have been advised that the doctor will be asked to discuss this with you at the appointment, which has been arranged to review your needs.


Once again I am sorry for the incorrect information I gave in my previous e-mail.

Dear Sir/Madam


A month nearly has gone by since I was informed by Britton House that I am being held on Active Review Process following the examination of my mental state at the appointment with Michael and Dr Simon (Gracious?) Issacs, and since then I had two telephone calls from Britton House, one from Amanda and the other from a nurse. I telephoned back three times to try and speak to Dr Odesanya or anyone else supervising the Active Review Process speaking to Jenny and others, but no one has telephoned me back to let me know what the process involves, whether I need to be present at Britton House of whether this will be a paper exercise by the Active Review Team


There is an appointment tentatively given to me of 8 September 2021 at 12.15 pm with Dr Odesanya, the Consultant Psychiatrist, for a Medication Review so I would like to have a Report on the Active Review Process before I attend this appointment. I am hereby drawing to the Community Mental Health Team's attention the attached summary of my present involvements for consideration at the Active Review Process: A River has small beginnings.docx.



Please acknowledge receipt of this email so that I know what is going on, as the uncertainty is causing me mental distress for which I am copying this email to the Samaritans to assuage my emotions.
